Why Shoulder Pain Often Comes on Overnight

A very common scenario involves strain to the supraspinatus tendon one of the key rotator cuff tendons. This may occur during lifting, training or repetitive use. After the initial strain:

  • The tendon becomes irritated

  • Inflammation increases overnight

  • The shoulder “gels” while you sleep

As inflammation builds, the subacromial bursa becomes irritated, leading to classic symptoms of shoulder bursitis. The result is often a sudden inability to lift the arm overhead or out to the side, accompanied by sharp pain.


Common Gym-Related Causes of Shoulder Injury

Exercise-related shoulder pain is extremely common, particularly when fatigue, technique breakdown, or excessive load is involved. The most frequent gym-related contributors we see include:

  • Shoulder press (especially overhead pressing under fatigue)

  • Single-arm rows where the shoulder is allowed to drift too far forward into excessive stretch

  • Chest flys, which can overstretch the anterior shoulder capsule

  • Farmer’s Carry that tractions the supraspinatus tendon

Over time, these stresses can overload the rotator cuff tendons and surrounding structures, leading to pain, weakness and inflammation.


Rotator Cuff Injury vs Bursitis – Getting the Diagnosis Right

Shoulder pain may arise from:

  • Rotator cuff tendinopathy or tearing

  • Subacromial bursitis

  • Shoulder impingement

  • Joint and postural dysfunction involving the neck or upper back

Accurate diagnosis is essential. Ultrasound imaging is often recommended to assess rotator cuff tendons and bursae. If symptoms fail to respond or more serious injury is suspected MRI may be required.

Should conservative care not improve your symptoms significantly in the short term, then a cortisone into the sub-acromial bursa will often give you great relief as well as preventing deterioration into a Frozen Shoulder.

Rest alone is rarely the answer. Once pain settles, progressive loading and strengthening is essential to restore tendon health, shoulder stability, and confidence in movement. Returning to exercise too quickly or avoiding it altogether can both prolong recovery.
