What Is Sciatica?
Sciatica is a condition caused by pressure or irritation of a spinal nerve root, most commonly in the lower back. When this occurs, pain can travel from the lower spine through the buttock and down one leg, sometimes reaching the calf or foot.
Symptoms may include:
Sharp, burning, or electric-like leg pain
Numbness or tingling in the leg or foot
Muscle weakness
Pain that worsens with sitting, bending, or coughing
The most common causes of sciatica include disc bulges or disc herniations, spinal joint dysfunction, inflammation around the nerve, or narrowing of the spinal canal.
When Sciatica Becomes Serious
In rare but serious cases, severe nerve compression can progress to a condition called cauda equina syndrome. This is a medical emergency and may present with:
Loss of bowel or bladder control
Saddle numbness
Progressive leg weakness
If cauda equina is suspected, urgent surgical decompression is required to prevent permanent nerve damage. Recognising these red flags early is critical.

Emergency Cauda Equina Referral
A patient who had already consulted hospital doctors presented in severe distress. Dr Cappon identified signs of cauda equina syndrome and immediately referred him to Sir Charles Gairdner Hospital. The patient underwent decompression surgery the next day, preventing permanent damage. His wife later left us an outstanding review praising the rapid and decisive care.Sciatica That Wasn’t a Disc Problem
Another patient was booked for a nerve root sleeve injection. On review, Dr Cappon noted that the MRI findings were relatively mild, yet the symptoms were extreme. Using advanced clinical assessment, the true cause was identified as vascular claudication, not a disc bulge. The patient was urgently referred for ultrasound and underwent iliac artery stenting surgery, restoring blood flow to his legs.
